Non-compliance with procedures leads to fall onboard Shanghai Spirit

The Australian Transportation Safety Bureau (ATSB) issued an investigation report on a serious injury of two crew members onboard the Hong Kong-flagged bulk carrier ‘Shanghai Spirit’, northeast of Port Alma, Queensland. The ATSB found that, contrary to established procedures, the two crew remained on the unsecured scaffold tower in preparation for repositioning, rendering it top-heavy and unstable.

Yantian Express cargo still waits to be discharged

Ocean Network Express published an advisory release, according to which fire-stricken Yantian Express is still waiting to discharge the damaged containers, two weeks after arriving in Freeport, Bahamas. The vessel is expecting the port Authority to give the official approval to begin the discharge operations.

According to the statement, the approval is expected to be received in the next few days.

Two workers died after fire on tanker in Chittagong

A fire broke out in the engine room on board a tanker, beached in Chittagong, on February 18, killing two workers. According to information, the two men died while scrapping the ship Greek Warrior at Shagorika Ship Breaking Yard.

One of the men was burned and rushed to Chittagong Medical College Hospital, but he died before reaching the hospital. The body of the second man was discovered on board a few hours later.

Insufficient risk management led cargo ship to contact wharf

ATSB published its final report into the cargo ship ‘Madang Coast’ impacting a wharf at the Port of Townsville. The incident has led to the updating and full implementation of a safety management system, in order to better identify risks and how to mitigate them.

Fatal fall from accommodation ladder attributed to poor safety culture

The Government of Luxembourg’s Administration of Technical Investigations (AET) issued an investigation report on the fatal occupational accident onboard the chemical oil tanker Nabucco on 26 June 2017.
