Real Life Incident: Able-Bodied Seaman Dies Loading Logs On Bulk Carrier

At approximately 2130 on 2 April 2019, the bulk carrier Coresky OL completed loading logs in Eastland Port, Gisborne. Shortly afterwards the crew commenced securing the logs that were stowed on deck.

At 2341 the chief officer, boatswain and two able-bodied seamen had just completed securing the logs stowed on top of number one hatch and were commencing to lash the logs stowed on top of number two hatch. It was dark but the deck was well lit by both the ship and shore floodlights.

At about 0035 on 1 June 2018, YM Efficiency was en route to Sydney, steaming slowly into strong gale force winds and very rough seas off Newcastle when it suddenly rolled heavily.

At about 0127 on 20 January 2018, the then UK flagged1 container ship CMA CGM G. Washington unexpectedly rolled 20° to starboard, paused for several seconds, then rolled 20° to port.

The ship was experiencing very heavy seas in the North Pacific Ocean while on passage from Xiamen, China to Los Angeles, USA.

In daylight the following morning, the crew found that three container bays had collapsed, with 137 containers lost overboard and a further 85 damaged.

Case Study: Vessel Drags Anchor Causing Multiple Collisions

Catamaran Capsizes In Rough Seas Off Newcastle Coast; 2 Rescued, 3 Dead

AMSA has completed search and rescue operations following the capsize of a catamaran in rough seas off Newcastle with five people on board, including a teenage girl.

Two people were rescued from the water and taken to John Hunter Hospital for assessment, a 14-year old girl and 45 year-old man. Three people were sadly confirmed deceased at the scene and bodies were recovered from the water by Volunteer Marine Rescue NSW.
