Lessons Learned: Man Overboard Cargo Vessel

Bahamas Maritime Authority reports of a man overboard incident.

What happened

On the evening of 28 March 2024, the Bahamas flagged coastal general cargo vessel was outbound from Groveport, UK. After rigging the pilot ladder, one seafarer returned to the accommodation whilst the other stayed on deck to smoke a cigarette. Around fifteen minutes later the master called the seafarer on the handheld radio to say that the pilot was ready to disembark but got no reply. Having called several times, the...

Lessons Learned: Cargo Ship Stuck in River After Tide Falls

Reasons why a cargo ship got stuck in a Cambridgeshire river have been outlined in a preliminary assessment by the Marine Accident Investigation Branch (MAIB). A vessel ran aground near New Dyke Farm on the River Nene while en route to Wisbech on June 25 this year.

Synopsis

At 0848 on 25 June 2024, the St Kitts and Nevis registered general cargo vessel Baltic Arrow ran aground near New Dyke Farm on the River Nene while enroute to Wisbech, England. There was no damage or pollution and no one was...

Lessons Learned: Crew Suffer Injury To Little Finger

IMCA reports that a worker suffered an injury while painting an auxiliary winch drum following maintenance.

What happened

While performing this work a wooden beam was used to prevent the drum from turning freely. Two painters, along with their supervisor, inspected their work area and conducted a toolbox talk before beginning to clean the winch drum base. While cleaning the auxiliary winch drum base, the painters needed to rotate the drum for better access. The two painters manually rotated the...

Unauthorized Clamp on Vessel Poses Safety And Environmental Risks

During a client inspection, a vessel’s main engine lubricating oil storage tank was found with an unauthorized homemade clamp that kept the self-closing valve open, violating safety regulations and standards. The modification was made to save time during routine tasks but created significant safety, fire, and environmental hazards. No prior inspections or observations flagged the issue. Corrective actions included searching for similar unauthorized devices, reinforcing safety protocols, and...

Lessons Learned: Laceration To Right Index Finger

The Energeo Alliance (formerly IAGC) has published a safety alert relating to a crew member who suffered a laceration to the right index finger while going through a vessel door, reports IMCA.

What happened

The worker was entering a workshop container located on deck. They opened the door with their right hand and stepped inside holding the door with their right hand to close it behind him. As the door closed, the wind and the movement of the vessel combined to cause the door to close more...

Lessons Learned: Non-fatal Man Overboard

IMCA reports of a non-fatal incident where a worker fell from height into the sea.

What happened

A worker fell 3m from a new jacket into the sea. The worker was uninjured and successfully recovered without harm. The incident occurred during the installation of a new jacket. During the operations, a worker accessed an area on the jacket which did not have any grating or scaffolding. Whilst exposed to the hazard (Working at height) but trying to secure himself to the guide rope, the worker fell...

Lifeboat Air Cylinder Explosion on An Empty Installation

An air bottle exploded in a lifeboat during an inspection of an uncrewed offshore platform, causing significant damage. The explosion was discovered while the platform was shut in, and no injuries or fatalities occurred since there were no personnel on board. The explosion caused one air bottle to propel upward into the lifeboat’s ceiling, another to fall into the sea, and one to remain intact. The lifeboat was later removed for further investigation, reports IMCA.

What happened

An air bottle...

Lessons Learned: Person Fell From Step Ladder

IMCA reports of an incident where a person fell off a 2-step ladder.

What happened

A worker fell off a 2-step ladder/hop up and was injured resulting in an LTI. A tool required some maintenance before going into storage after use. To gain access to the tool and comfortably carry out the task, the worker needed to stand on a 2-step ladder/hop up.

While the worker was on the 2-step ladder/hop up, the vessel hit a wave and pitched, and this caused the worker to lose balance and fall. The step...

Seaman Injured by Entangled Mooring Rope During Routine Ferry Operation

As the able seaman (AB) on the forward mooring deck prepared to release the last of the mooring ropes, he noticed that the heaving line had become entangled with the middle section of the mooring rope, and the loose end had been tied off on the handrail. Recognizing the potential hazard, the AB released the heaving line and the mooring rope, carefully paying them out together until the linesman ashore had enough slack to release the mooring rope. Once the linesman let go of the mooring rope,...

Quick Crew Action Prevents Major Fire after Oil Leak on New Cargo Vessel

A newly built specialist cargo vessel narrowly avoided a major fire after a temperature probe in its main propulsion system gearbox detached during a voyage, leading to a significant oil leak. Operating at 25 bar, the oil was ejected forcefully from the gearbox, creating an oil jet that sprayed a large portion of the engine room, including a running main engine, reports MAIB.

Incident Overview

The oil leak occurred when the temperature probe, held in place by a pipe compression fitting, detached...
